Request for proof of census registration: certificates and flyers

Certificate

The certificate certifies residence and habitual residence. You must apply in case you have to submit it to the following agencies:

  • Judicial bodies (courts and tribunals)
  • Foreign military agencies or authorities
  • Civil Registry (marriages, nationality, changes of name and/or surnames and adoptions, etc.) Declarations of heirs
  • Other Official Registries, Prisons, Customs Office, SEPE, Universities
  • Residence due to roots (three years of residence is required).
  • Recognition of dependency status

Flyer

This document is for information purposes and indicates the residence and usual domicile. It will not be necessary to provide this document if the interested party AUTHORIZES the corresponding Administration to verify his or her census address. It is requested, among others, in the following procedures:

  • For the issuance of the National Identity Card
  • Vehicle registration
  • Swimming pool vouchers, Social Services subsidies
  • Procedures in which the certificate is not expressly required, etc.

 

Documentation

  • Face-to-face requests:
    • DNI, or Passport, or Foreigner’s Card, or Identity Card in case of being a national of the European Union, of the interested party.
    • In the case of historical certificates, an application form will be provided at the Citizen Service Offices. It can also be downloaded from the section ‘Form Templates’.
    • In the case of unemancipated minors:
      • Applicants (whether parents or not) are registered in the registry where the minor is registered: original of DNI, or Passport, or Foreigner’s Card, or Identity Card in case of being a national of member states of the European Union, of the applicants.
      • Applicants (whether parents or not) are not registered in the registry where the minor is registered: original of DNI, or Passport, or Foreigner’s Card, or Identity Card in case of being a national of member states of the European Union, of the applicants and authorization of the adult with whom the minor is registered, attaching a photocopy of the identity document of the authorizing party.
    • In the case of deceased persons: DNI, or Passport, or Foreigner’s Card, or Identity Card. in case of being a national of the European Union, both of the deceased and of the applicant, in all cases, the Death Certificate issued by the Civil Registry or the page of the Family Book in which the death is registered, as well as the document of accreditation of legitimate interest, such as, among others, must also be presented:
      • If the person requesting it is the spouse: Family Book or Marriage Certificate.
      • If the applicant is a child or parent: Family Book or Birth Certificate.
      • If the applicant is a testamentary heir: Will and/or Certificate of Last Will and Testament.
      • If the applicant is not registered at the same address as the deceased person, nor is he/she listed as heir, he/she must provide a document accrediting his/her status as interested party or representative of the interested party.
      • When the applicant is a legal entity (e.g., an agency) or other professionals (e.g., a lawyer), photocopies of the documents proving that they are acting on behalf of the legitimate interested parties must be provided.
    • Requests by mail:
      • Written request signed by the interested party,indicating:
        • What is requested
        • Name and surname
        • Photocopy of the DNI, or Passport, or Foreigner’s Card, or Identity Card in case of being a national of the European Union, of the interested party.
        • Complete mailing address where you wish to receive the information
        • Contact telephone number
      • In the case of unemancipated minors:
        • Applicants (whether parents or not) are registered in the registry in which the minor is registered: photocopy of ID card, or Passport, or Foreigner’s Card, or Identity Card in case of being a national of member states of the European Union, of the applicants.
        • Applicants (whether parents or not) are not registered in the registry where the minor is registered: photocopy of DNI, or Passport, or Foreigner’s Card, or Identity Card in case of being a national of member states of the European Union, of the applicants and authorization of the adult with whom the minor is registered, attaching a photocopy of the identity document of the authorizing party.
      • In the case of deceased persons : Photocopy of DNI, or Passport, or Foreigner’s Card, or Identity Card in case of being a national of the European Union, both of the deceased and of the applicant, in all cases, the Death Certificate issued by the Civil Registry or the page of the Family Book in which the death is registered, as well as the document of accreditation of legitimate interest, such as, among others, must also be presented:
